Dr. Carrie Hsia, DC

Dr. Hsia (Pronounced *SHAW) is a 2016 graduate from the University of Western States in Portland. She has worked in a variety of clinics including not for profit organizations. She was the 2017 Chiropractic partner for the Rose City Rollers and has worked with different types of athletes and artists. Dr. Hsia also teaches students and other practitioners on cultural competency. Being at the Jubitz location for over a year, Dr. Hsia prides herself in helping the drivers with their health and wellbeing keeping everybody on the roads safe. She is a certified DOT/CDL NRCME Medical Examiner.

 

Dr. Justin Knight, DC

Dr. Justin Knight is a 2013 graduate from the University of Western States in Portland, Oregon and is a certified DOT/CDL NRCME Medical Examiner. Prior to receiving his doctorate in Chiropractic he graduated from Portland State University with honors in Human Biology and practiced Licensed Massage Therapy for a number of years in Oregon.

His background of bodywork, knowledge of human anatomy and physiology and continued post-doctorate education has helped him to have a deep and broad understanding of the musculoskeletal system and associated structures. This has allowed him to develop a holistic approach to physical medicine that focuses on each individual's unique needs in order to obtain their optimal health and function.

He has a 5 year old son Lucas who is the light of his life. In his time off he enjoys camping, hiking, biking, rafting, skiing...pretty much everything the Northwest has to offer.

DOT Physical Exam Checklist

Everyone needs to bring:

  • • State-issued photo ID. Best option is your driver’s license.
  • Prepared to provide a urine sample to test for kidney functions and protein levels

You may also need to bring:

  • • Glasses or contact lenses if you wear them for driving or work.
  • • Working hearing aid(s) if you use them.
  • • “Medical Release” documentation and/ or lab results from your treating physician for any serious medical condition including diabetes, stroke, heart disease and cancer.
  • • CPAP readout or chip if you use one for sleep apnea.
  • • Medication list
  • • Federal Waiver, SPE Certificate, or Exemption Certificate if you need one.

Please make sure you do not:

  • • Chew or use tobacco directly before or during the exam.
  • • Drink excessive beverages containing caffeine before the exam.
  • • Use cell phone during exam.
