Consortium Member Test Authorization Form

This form can be filled out by companies already enrolled in the OTA Consortium. If  you have not enrolled into the OTA Consortium, please visit the OTA Consortium Membership page and fill out the interest form.

Quick Tips on Filling out this form: 

• DOT Testing = If you are wanting to authorize a Department of Transportation test, please ensure the employee has a valid CDL or permit
• Non-DOT Testing = If you are wanting to authorize any test not required by federal regulations
